Model’s permission:

 

I, as named above, hereby give Novartis permission to use the images and their audio and visual content free of charge (except if explicitly agreed otherwise in a separate written agreement) and in any Media for any Novartis related purpose which may include, among others, Novartis Internet activities, and internal and external social media channels (e.g., Facebook, etc.), advertising, promotion and marketing, and waive any right to inspect or approve any final product. I agree that the images may be combined with other images, sound, text, graphics, and cropped/cut, altered or modified. I acknowledge that the images may be published on the Internet. The images shall include my story and any photographs and videos of it, including spoken language. 

 

I represent and warrant that I am 18 years or older and that I have understood the content above.
